Where to File:

SSS branch nearest the members place of death or to the office of the nearest accredited life-plan company in case he wants to avail of the Memorial Service Assistance Program (MSAP)

How to File:

The person who shoulders the burial expenses of a deceased SSS member or pensioner may apply for a funeral grant in the amount of P20,000. He must accomplish one copy of the Claim for Funeral Benefit Form (BPN-103) using black ink and submit it to the SSS branch nearest the member's place of death. He may also choose to avail of the Memorial Service Assistance Program (MSAP), in which case he should submit Form BPN-103 at the office of the nearest accredited life-plan company.

The claimant must also accomplish a Filer's Affidavit (Sinumpaang Salaysay), to which he must affix a recent 1" x 1" photograph. If the member's cause of death is work-related, the claimant must also submit a report of death, duly accomplished by the member's employer.

1) Sinumpaang Salaysay affixed with a recent 1x1 photograph

2) Report of death duly accomplished by the members employer
