How to File:

The Specimen Signature Card (Form L-501) can be used by an employer to indicate or to amend the list of company officials authorized to certify or sign documents and forms intended for the SSS.

The employer's authorized representative must fill up Form L-501 using black ink and submit it in two original copies to the nearest SSS branch. The first page must be printed on thick paper, such as white oslo paper or uncolored (white) board.
