How to File:
The Contribution Collection List (Form
R-3) is used by an employer-member to report his employees' Social Security and Employee's Compensation contributions. The form must contain the names of the employees and their corresponding SS numbers.
You must fill up Form R-3 using black ink and submit it in two copies in person or by mail to the nearest SSS Branch. See page 2 of the form for the instructions. If you are sending the R-3 form by mail, please be sure to mail the R-5s and SBRs along with it.
