How to File:

All applicants shall file their application form with the Licensing Division, Insurance Commission in UN Avenue, Manila.

Other Documentary Requirements:

1) Original License

2) Proof of Filing of Quarterly Report (Clearance from Rating)

3) Renewed Surety Bond for each line

4) Fire Tax Receipt (with fire line only)

5) ITR of the company or corporation

6) Audited Financial Statement

7) ITR of all adjusters

8) PTR of all adjusters

9) Value Added Tax (VAT) Receipts or Percentage Tax

10) Documentary Stamp

11) Updated list of Officers, Board of Directors and Stockholders and Companys Capital Structure
