o For private laboratories: Certificate of Registration of Business Name from the Department of Trade and Industry and certificate from the Securities and Exchange Commission, if a corporation.
o Certificate/s from the Philippine Board of Pathology
o PRC Board Certificate of Medical Technologists
o A notarized statement that the applicant has complied with all business requirements under existing laws or ordinances in the locality where the laboratory is located.
