ELIGIBILITY REQUIREMENTS FOR REGISTRATION
The Bids and Awards Committee 1 (BAC1) will process contractor’s application for
registration
with
complete
requirements/supporting documents and
will
correspondingly issue the Contractor’s Registration Certificate (CRC).
All reproduced copies or Xeroxed copies must be originally signed true copies by
authorized signatories.
The Contractor must provide tabs or ears for easier checking.
To expedite the processing of the CRC, the Contractor may submit an ecopy of the
information in a CD, USB or may email the same to nha_bac@yahoo.com the Lists of
Completed Projects, On-going Projects, List of Personnel and List of Equipment.
Only prescribed forms should be used.
A prospective bidder must submit the following eligibility documents for registration:

A.

LEGAL DOCUMENTS:
1.

Contractor’s Data
a) registered name
b) address
c) telephone and fax numbers
d) e-mail address
e) Person Managing the Affairs of the Firm (name, designation, cell or
phone numbers)
f) Three (3) Signature Specimen of authorized signatory of the firm.
g) Authorized liaison officer/s (name, designation, cell or phone numbers
and respective signatures).
NOTE: All official communications (oral or written) from the BAC1 shall
be sent only through these addresses and phone numbers and to the
authorized signatory and liaison officer/s. Any inquiry by any
unauthorized persons shall not be entertained by the BAC1.

2.

The authorized signatory and liaison officer/s must have written authority
from the Board of Directors or Secretary’s Certificate to sign and represent
the firm in the bidding of projects.

3.

Registration Certificate from the Securities and Exchange Commission
(SEC) supported with Articles of Incorporation and By-Laws, all
Amendments, and Increase in Capitalization with attachments; Department
of Trade and Industry (DTI) for sole proprietorship; or Cooperative
Development Authority (CDA) for cooperatives.

4.

Mayor’s Permit issued by the city or municipality where the principal place
of business of the prospective bidder is located supported with an Official
Receipt (O.R.) evidencing payment for the latest applicable quarter or
whole year 2015.

5.

Valid Certificate of PhilGEPS Registration.

6.

Valid signed Philippine Contractors Accreditation Board (PCAB)
License and Registration.

7.

Omnibus Sworn Statement using the form attached herein to include
statement using the prescribed form for:
a)
b)
c)
d)
e)
f)
g)
h)
i)

Ownership/corporation;
Authority of signatory;
Not “blacklisted” or barred from bidding;
Authenticity of documents;
Authority granted to NHA to verify all the documents submitted;
Disclosure of no relationship;
Compliance with existing labor laws and standards;
Bidder’s responsibilities for NHA projects; and
Non-payment of pecuniary interests.

C. FINANCIAL DOCUMENTS
8.

Audited financial statements of the Contractor among others, the prospective
total and current assets and liabilities, stamped “received” by the BIR or its
duly accredited and authorized institutions, for the immediately preceding
calendar year, including among others, the:

Filing and Payment System (EFPS) for the last applicable six (6) months
duly validated with the tax payments made thereon pursuant to BIR
Regulations No. 3-2005 dated February 16, 2005.
11. Tax Clearance valid for the current year issued by the BIR to prove full and
timely payment of taxes pursuant to EO 398.

Certificate of Value-Added Tax (VAT) Registration

TECHNICAL DOCUMENTS
14. Statement of all its COMPLETED government and private contracts using
Form SF-INFR-16. The statement shall include, for each contract, the
following:

Nature of work;

The statement shall be supported with Contracts/Agreements, Notices
of Award and/or Notices to Proceed issued by the owners; and
The statement shall be supported with Certificate of Owner’s
Acceptance; and/or Constructors Performance Evaluation Summary
(CPES) Final Rating, and/or Certificate of Satisfactory Completion.

15. Statement of all its ON-GOING government and private contracts, including
contracts awarded but not yet started, if any, using Form SF- INFR-15. The
statement shall include, for each contract, the following:

16. Organizational Chart of the firm showing specific names of owners, key
technical staff, personnel and their positions/designations (This refers to
the firm and not to a specific project).
17. List of contractor’s personnel (viz, Project Manager, Project Engineers,
Materials, Electrical, Sanitary, Geodetic, and Cost Engineers,
Construction Safety Engineer or Officer and Foremen, etc.) using Form SFINFR-48, supported w i t h their complete qualification and experience
data sheets, PRC Licenses, and Certificate of Training for the Safety
Engineers/Officers i s s u e d b y BW C/DOLE o r BWC/DOLE-accredited
training centers for basic occupational training (This refers to the firm and
not to a specific project) to determine the firm’s technical capability to
undertake NHA projects.
18. List of contractor’s equipment units, which are owned, leased, and/or
under purchase agreements, appropriately supported with proofs of
ownership such as Official Receipts of purchase/acquisition, Deeds of Sale,
Lease Contracts, and/or Certification of Availability of Equipment from the
equipment lessor/vendor using Form SF-INFR-49 to determine the firm’s
technical capability to undertake NHA projects (This refers to the firm and
not to a specific project).

PROCESSES TO BE FOLLOWED IN THE REGISTRATION OF CONTRACTORS:
1.

The BAC1 had published the Invitation to Apply for Registration in a newspaper of
general nationwide circulation and had posted within 14 calendar days in the
PhilGEPS and NHA websites and in conspicuous places in the NHA premises.

2.

Interested Contractors may submit complete eligibility requirements to BAC1 for
registration. The BAC1 will process contractor’s application for registration with
complete requirements/supporting documents.

3.

The BAC1 reviews the documentary requirements and determines if the
requirements are fully complied with.

4.

The BAC1 shall undertake prequalification of the Contractor on the basis of
its submitted documents.

5.

The BAC1 issues the Contractor’s Registration Certificate (CRC) and Contractor’s
Profile to Contractors who satisfy the criteria for registration.

6.

The CRCs shall remain valid for a period of one (1) year provided that the
documentary requirements are maintained current and updated at all times.

7.

If a renewal is desired, application for the same shall be filed at least thirty (30)
days before the expiration of the Certification.

It is the responsibility of the Contractor to determine if its CRC and Contractor’s
Profile are maintained current and updated. The Contractor may request for the
issuance of updated Contractor’s Profile upon submission of updated documents.

9.

The C ertification is personal, non-transferable and may be renewed only at the
instance of the Contractor concerned upon its expiration. Any misuse hereof
shall cause the forfeiture of the established right and consequent debarment of
the contractor.

10. During bidding of projects, the BAC1 may accept the CRC together with the
Contractor’s Profile in lieu of the Eligibility requirements.
11. An unregistered bidder may apply for the issuance of the CRC by submitting a
written letter of intent to the BAC1 on or before the deadline for the submission of
the letter of intent to give time to process the application. Any application for
eligibility or updates submitted after the deadline for the submission of the LOI
shall not be considered for the bidding at hand.
12. Registration of interested Contractors shall be undertaken on a continuing basis.
The registered Contractor may update its documentary requirement/s anytime as
the need arises.
13. The processing of documents shall be done on a “first come, first serve
basis”. The prospective should give ample time for the processing of applications.
14. It is the responsibility of the applicants for registration to inquire or follow-up from
the BAC1 the status of their application for registration. Applicants who do not
qualify for registration for the first time may submit the required documents to
completely satisfy the requirements.
15. The Contractor shall submit all documents neatly and sequentially filed as
written in the list in a thick folder which shall serve as the mother file of the
Contractor.
18. The forms may be downloaded from the NHA website at www.nha.gov.ph. Only
prescribed forms should be used.
20. The Contractor shall submit a Letter of Intent for registration and submit the
complete documents addressed to:
