DOCUMENTARY REQUIREMENTS FOR REGISTRATION OF
SUPPLIERS, MANUFACTURERS & DISTRIBUTORS (Goods)
A prospective bidder/supplier/manufacturer/distributor must submit the following
documents for registration. Only applications with complete supporting documents shall
be processed by the Bids and Awards Committee 1 (BAC 1).
ELIGIBILITY DOCUMENTS (Class "A" Documents):
A.
Legal Documents:
1.
Bidder’s/Supplier’s/Manufacturer’s/Distributor’s
data
(registered
name, address, telephone and fax numbers, and email address), Person
Managing the Affairs of the Firm (name, designation and phone numbers)
and authorized liaison officer/s (name, designation and phone
numbers). The NHA BAC 1 shall transact business only with the
authorized signatory and/or authorized liaison officer/s. The authorized
liaison officer/s shall present at all times its authority when transacting
business with NHA BAC 1.
All official communications from the BAC shall only be sent through these
addresses and phone numbers only.
2.
Three (3) Signature Specimen of the Authorized Signatory of the firm.
Ownership/Corporation should be appropriately supported with
Corporate Secretary’s Certificate or other documents designating the
authorized signatory and/or participation of the firm in the bidding.
The authorized representative must have Notarized Authority from the
Bidder/Authorized Signatory to submit bids, as follows:
FOR CORPORATION: Notarized Corporate Secretary’s Certificate; and
FOR SOLE PROPRIETORSHIP:
Proprietor/Owner.

Registration Certificate from the Securities and Exchange Commission
(SEC) supported with Articles of Incorporation, Amendments, and
Increase in Capitalization, or from Department of Trade and Industry
(DTI) for sole proprietorship, or from Cooperative Development Authority
(CDA) for cooperatives.

Mayor’s Permit issued by the city or municipality where the principal place
of business of the prospective bidder is located supported by an Official
Receipt (OR) evidencing payment for the latest applicable quarter or
whole year of 2014.

Tax Clearance for the current year issued by the BIR to prove full and
timely payment of taxes pursuant to EO 398.
7.
Taxpayer’s Identification Number (TIN).
8.
Certificate of Value-Added Tax (VAT) Registration.

Valid Certificate of PhilGEPS Registration.
Duly notarized Omnibus Sworn statement (general statement for NHA
projects) using the form attached herein to include statement using
prescribed form for:
a) ownership/corporation;
b) authority of signatory;
c. not “blacklisted” or barred from bidding;
d. authenticity of documents;
e. authority granted to NHA to verify all the documents submitted;
f.
disclosure of no relationship;
g. compliance with existing labor laws and standards; and
h. bidder’s responsibilities.

Technical Documents
10.
Statement of all its COMPLETED government and private contracts using
FORM SF-GOODS-13b. The statement shall include, for each contract,
the following:

End user’s acceptance or Official Receipt/s issued for completed
contracts.
Attach copies of Notices of Award and/or to Proceed, contract agreement
or purchase orders of completed contracts and certificate of completion
and acceptance, if applicable.
11.
Statement of all its ONGOING Government and private contracts,
including contracts awarded but not yet started, if any, using Form SFGOODS-13a. The statement shall include, for each contract, the
following:

Amount of contract and value of outstanding contracts; and
Date of delivery.

Attach copies of Notices to Proceed, contract agreement or purchase
orders of ongoing contracts.
C.
Financial Documents
12.
Audited Financial Statements of the Contractor - these include among
others, the: (a) Independent Auditor’s Report (IN MY OPINION….); (b)
Income Tax Return; (c) Balance Sheet; and (d) Income Statement, and
showing, among others, the prospective total and current assets and
liabilities, stamped “received” by the BIR or its duly accredited and
authorized institutions, for two (2) preceding calendar years.

Business Tax Returns paid thru the Electronic Filing and Payment
System (EFPS) for the last applicable six (6) months duly validated
with the tax payments made thereon pursuant to BIR Regulations No. 32005 dated February 16, 2005.
TECHNICAL DOCUMENTS –
15.
(For rental of vehicles and transport services) List of Supplier’s
vehicles, which are owned, leased, and/or under purchase agreements,
appropriately supported by proofs of ownership such as Official Receipts
of purchase/acquisition, Deeds of Sale, Lease Contracts, and/or
Certificate of Availability of Vehicles from the vehicle lessor/vendor using
Form Vehicle List.
PROCESS OF REGISTRATION
1.
The BAC publishes the Invitation to Apply for Registration in a newspaper of
general nationwide circulation and posted within 14 calendar days in the
PhilGEPS and NHA websites and in conspicuous places in the NHA premises.
2.
Interested Bidder/Supplier/Manufacturer/Distributor must submit complete
eligibility requirements to BAC for registration. Only applications with complete
documents shall be processed by the BAC.
3.
The BAC reviews the documentary requirements and determines the
Contractors who have fully complied with the requirements.
4.
The BAC shall undertake eligibility evaluation of the documents submitted by the
Bidder/ Supplier/ Manufacturer/ Distributor only on the basis of its submitted
documents.
5.
The BAC issues the Certificate of Registration to the Bidder/ Supplier/
Manufacturer/ Distributor who satisfies the criteria of registration.
6.
The Certificate of Registration shall remain valid for a period of one (1) year from
the issuance of the Certificate of Registration, provided that the documentary
requirements are maintained current and updated at all times. If a renewal is

desired, application for the same shall be filed at least thirty (30) days before the
expiration of the Certificate of Registration.
7.
The certification is personal, non-transferable and may be renewed only at the
instance of the contractor concerned upon its expiration. Any misuse thereof
shall cause the forfeiture of the established right and consequent delisting of the
Bidder/ Supplier/ Manufacturer/ Distributor.
8.
The Certificate of Registration together with the Bidder/ Supplier/ Manufacturer/
Distributor’s Profile may be submitted to the NHA during bidding in lieu of the
Eligibility requirements.
9.
Notwithstanding the issued Certificate of Registration, an interested
Bidder/Supplier/Manufacturer/Distributor shall submit a written letter of intent
(LOI) and/or its application for eligibility to the BAC on or before the deadline
specified in the Invitation to Register. Any application for eligibility or updates
submitted after the deadline for the submission of the LOI shall not be
considered for the bidding at hand.
10.
The Registration of interested Bidder/Supplier/Manufacturer/Distributor shall be
undertaken on a continuing basis. The registered Bidder/ Supplier/
Manufacturer/ Distributor may update its documentary requirement/s anytime.
11.
All applicants may inquire of the status of their application. Applicants who do
not qualify for accreditation may submit the required documents to completely
satisfy the requirements.
12.
The processing of documents shall be done on a “first come, first serve
basis”.
13.
The Bidder/Supplier/Manufacturer/Distributor shall submit all documents neatly
and sequentially filed as listed/written in a thick folder and with tabs and ears
for easier checking of documents. The documents shall serve as the mother file
of the Contractor.
14.
All reproduced copies or Xerox copies must be certified true copies by the
bidder.
