Birth Certificate Birth Certificate duly authenticated by the Philippine Embassy
Alien Employment Permit from DOLE (AEP or SWP from BI whichever is applicable)
Certification of Employment of the Applicant issued by the President, Chairman of the company
that the applicant is an employee of a TIEZA Designated TEZ or Registered Tourism Enterprise

Medical Certification that the applicant is in good medical and psychological condition or results
