REQUIREMENTS FOR VISA ISSUANCE
A. PRINCIPAL / INVESTOR

Duly accomplished and notarized TIEZA SIRV Application Form
12 pcs 2 x 2 ID picture with blue background
Passport and Photocopy of Passport with latest arrival
Birth Certificate duly authenticated by Philippine Embassy
Proof of Investment
 Sworn Certification from the Investee Corporation indicating shares paid by the foreign
investor
6. Certificate of Inward Remittance under oath signed by the President or Sr. EVP or officer with a
rank not lower than Asst. Vice President or officer with equivalent rank, and proof of inward
remittances converted into peso
(Note: Requirement for the issuance of the Bangko Sentral Registration document of investment)
7. NBI Clearance or Police Clearance
8. Medical Certification that the applicant is in good medical and psychological condition or results
of medical and psychological examinations (psychological examination is not required for
dependent children below 18 years old) by Philippine government accredited hospital / clinic or
by a private hospital duly notarized

2. Passport and Photocopy of Passport with latest arrival
3. Birth Certificate (Children) / Marriage Certificate (Spouse) authenticated by the Philippine
Embassy at the applicant’s country of origin
4. NBI Clearance or Police Clearance
5. Medical Certification that the dependents are in good medical and psychological condition or
results of medical and psychological examinations (psychological examination is not required for
dependent children below 18 years old)by Philippine government accredited hospital / clinic or
by a private hospital duly notarized

Birth Certificate Birth Certificate duly authenticated by the Philippine Embassy
Alien Employment Permit from DOLE (AEP or SWP from BI whichever is applicable)
Certification of Employment of the Applicant issued by the President, Chairman of the company
that the applicant is an employee of a TIEZA Designated TEZ or Registered Tourism Enterprise

9. Medical Certification that the applicant is in good medical and psychological condition or results
of medical and psychological examinations (psychological examination is not required for
dependent children below 18 years old)by Philippine government accredited hospital / clinic or

Employment Contract, Secretary’s Certificate of Election, Appointment, Assignment, Secondment
or Deployment of Applicant, or equivalent document indicating duration of employment,
compensation and other benefits, and scope of duties

6. Letter of Request from the petitioning company for issuance of Special Working Permit
