I, the undersigned and the President/Authorized Company Representative/Authorized

promise to attend the nearest scheduled *Meat Importer’s Orientation to be
conducted by the National Meat Inspection Service-Meat Import Export Assistance and
Inspection Division (NMIS-MIEAID) at NMIS Office, Visayas Avenue, Diliman, Quezon
City. I understood that attendance to this orientation is one of the requirements for
NMIS Accreditation of Meat Importers pursuant to DA AO No. 9, series of 2013.
More importantly, I am fully aware that, non-compliance to this requirement will
automatically result to the non-processing/issuance of my Certificate of Accreditation
and will eventually forfeit my application as Meat Importer.
