Letter of Intent;
Certified True Copy of Articles of Incorporation;
Certified True Copy of Registration Certificate from the Securities and Exchange
Commission (SEC)/ Department of Trade and Industries (DTI);
Certified True Copies of Audited Financial Statements for the last three (3) years;
Certified True Copies of Income Tax Returns for the last three (3) years;
Location sketch of office;
Others

Note:
All data/information reflected in the appraiser’s information sheet and the
attachments shall be kept and classified as “strictly confidential”.
