The fingerprints and signature specimen must be taken at the nearest station / office of the
Philippine National Police (PNP), National Bureau of Investigation (NBI) of Philippine Veterans Affairs
Office (PVAO) to avoid blurred / unreadable thumbprints. There must be NO ERASURES in the signatures
on the forms. All instructions must be followed strictly to avoid delay in processing your claim.
