The following quotation is based on Philippine Accreditation Bureau Schedule of Fees and the information
provided in the application documents.
Application Fee – (Non-refundable, payable upon issuance of application form or to
be included in the billing statement upon completion of the process) PhP 2,000.00
Documentation Review - ( Php 500.00 per man-hour)
Preliminary Assessment - (PhP 5,000.00 per man-day)
Initial Assessment (Stage 1 & 2) – (PhP 5,000.00 per man-day)
Follow-up visit, when necessary, to verify implementation of
corrective actions - (PhP 5,000.00 per man-day)
Accreditation Fee, payable upon issuance of Certificate – (PhP 10,000.00 per
Scope)
Annual fee on the first year, payable upon approval of accreditation – (PhP
5,000.00 per scope of accreditation)

TOTAL FEES
Surveillance visits conducted during the five-year
validity of the accreditation, PhP 5,000.00 per man-day
Annual Dues, payable within 15 days of billing, (PhP 5,000.00 per scope of
accreditation)
Actual costs of transportation and accommodation of personnel conducting assessment shall be
borne by the applicant body
