(3) DTI Certificate of Registration, if Corporation of Partnership, Certificate of Registration with
Securities and Exchange Commission (SEC) together with Articles of Incorporation and bylaws.
(4) Valid Mayor’s Permit
(5) BIR Registration and TIN
(6) SSS Certificate of Membership
(7) Proof of Financial Capacity or Audited Financial Statement
(8) Certificate of Registration and latest registration payments of MV used for practical driving lessons
