MONTHLY REPORT ON PRODUCTION, SALES, INVENTORY OF METALLIC MINERALS AND EMPLOYMENT DATA FOR GOLD

INSTRUCTIONS:
1. This form must be accomplished in duplicate.
2. Submit the original copy to MGB Central Office through the Mineral Economics, Information & Publications Division (MEIPD) , copy furnished the Concerned Region within 15 working days
at the end of each calendar month
3. Submission of this report is in compliance with SECTION 270, CHAPTER XXIX, OF DENR AO 96-40 or the Implementing Rules & Regulations of Republic Act No. 7942
4. Information to be supplied by the reporting Contractor/Operator should cover the period from the first day to the last day of the reporting month
5. This report form must be accomplished by Contractors/Operators producing GOLD as primary product.
6. Write within the spaces provided for your answers and do not leave any of the requirements unanswered. If a requirement does not apply to you, write NA (not applicable).
If the question is applicable to you but you have no answer, write NONE.
7. This document should be signed by the resident manager or equivalent position.
8. This report should be duly notarized.
9. For any comments, suggestions and clarifications regarding this form please consult your nearest MGB Regional Office in your area.
DEFINITIONS OF TERMS:
Mine Site refers to the location of mine where mining operations, specifically mineral extraction, takes place (Barangay, Municipality, Province)
Milling Capacity refers to the capacity of the mill used by the contractor/operator expressed in Ton Per Day (TPD).
Rated Capacity refers to the maximum fixed output of a mill or processing facility expressed in Tons Per Day (TPD). It is also the measure of the theoretical amount of material that
can be handled/recovered in any given time by the mill.
Actual Capacity refers to the attained capacity of the mill in Metric Tons Per Day (TPD). It also refers to the constant amount of material that can be recovered/handled in a given actual time per day.
Mill Recovery refers to the amount of valuable materials (metal/mineral product) obtained in an ore processing operation as compared to the total amount of waste material and is usually expressed as a percentage
of the total amount of ore processed
Milled Ore is also called "mill feed" or any ore that contains sufficient valuable minerals chosen to be further beneficiated.
Mined Ore refers to the total natural combination of minerals, especially one from which a metal like gold can be profitably extracted and expressed in DMT
Content refers to the grade of the ore or concentrate or processed products derived from operations.
Main Metal Concentrate refers to the final solid state of the product of the various metal extraction process.
Production quantity refers to the total volume of mineral extracted during the reporting period expressed in kilograms
Production value refers to the total value or gross output of mineral extracted during the reporting period as defined in Chapter 1, Section 5 of the Revised IRR of the Phil. Mining Act of 1995 (under the term "Gross Output)
Sales quantity refers to the total volume of mineral sold during the reporting period expressed in kilograms.
Sales value refers to the total value of mineral sold during the reporting period. This value includes all costs of production, taxes invoiced to purchasers, transport & trade mark-up.
Primary Product refers to the principal valuable mineral, metals or products obtainable in an ore mining and processing operations.
By Product refers to the secondary or additional valuable product obtainable in an ore mining and processing operations
Tailings the gangue and other refuse material resulting from washing, concentration or treatment of ground milling ore.
