QUARTERLY REPORT ON PRODUCTION AND SALES OF SMALL SCALE METALLIC
PRIMARY GOLD

INSTRUCTIONS:
1. This form must be accomplished by any mining operation having an annual production not exceeding 50,000 metric tons of run-of-mine ore, either an open cast mine
working or sub-surface men working.
2. Submit the original copy to your respective Provincial Governor/City Mayor, copy furnished MGB Central Office through the Mineral Economics, Information & Publications Division
and the Concerned Region within fifteen (15) days at the end of each calendar quarter
3. Submission of this report is in compliance with SECTION 270, CHAPTER XXIX, of DENR AO 96-40 or the implementing rules and regulataions of REPUBLIC ACT NO. 7942.
4. Information to be supplied by the reporting holder should cover the period from the first day to the last day of the report quarter.
5. This report form must be accomplished by a holder of a Small-Scale Mining Permit producing metallic minerals other than gold.
6. Write within the space provided for your answers and do not leave any of the requirements unanswered.
7. If a requirement does not apply to you, write NA (Not Applicable). If the question is applicable to you but you have no answer, write NONE.
8. This document should be signed by the Small Scale Mining Permittee.
9. This report should be duly notarized.
DEFINITION OF TERMS:
Small Scale Mining involves the operation of a single unit mining operation with a yearly production not exceeding 50,000 metric tons of run-of-mine ore. The working is artisanal
either open cast or shallow underground mining without the use of sophisticated mining equipment; a total capital investment not exceeding P10 million during the
effectivity of the permit and its renewal. A ration of labor cost to equipment utilization cost to produce, process and market one metric ton of ore equivalent to or
exceeding one (1) .
Mine Site refers to the location of mine where mining operations, specifically mineral extraction takes. place
Direct Shipping ore refers to that portion of mined ore that requires no beneficiation to pass market specifications, may only need some sort of washing or drying to be redied for selling
Content refers to the amount of gold on a per ton basis.
Component is usually expressed as a percentage of the total amount of ore processed
